Testing

Self-Assessment is a critical mental technique for developing your skills and assessing your own progress. It is recommended that you regularly use self-assessment after each game. This form assists you in answering the questions and is treated in confidence. You can have the option in forwarding your assessment to any coach/mentor of your choice. The secretary will maintain a copy for your well-being, and only release the information with permission.

All questions are optional.

In this self-assessment there are three areas of attention, Control, Decision Making and Communication. Free all to add any thing else that may be important to you. At the end of the questions, a report will be submitted back to you for you to check your progress.
